UNDERSTANDING CONTINUITY OF CARE FOR PATIENTS WITH ACUTE AND CRITICAL CONDITIONS: IMPROVING REHABILITATION PATHWAYS IN UKRAINE

Programme: Seed Funding Twinning Initiative

The Health Sciences Research Foundation supports the research and analytical component of the project: the methodological examination of approaches to analysing rehabilitation pathways, contributing to a systemic vision of continuity of care, and strengthening the research perspective for future international initiatives. The Foundation participates in joint working sessions with partners, discussing existing regulatory, programme and organisational approaches to acute rehabilitation and the subsequent referral of patients. The expected outcomes of the project include the preparation of a joint analytical and conceptual document, the identification of gaps at key transition points in rehabilitation pathways, and the development of a roadmap for further research, educational collaboration and international partnership.

Goal: To strengthen the evidence base and analytical framework for the development of the rehabilitation system by analysing patient pathways, identifying gaps in the continuity of care, and developing practical approaches to its further improvement.

Period: April 2026 – February 2027
